Conditions of the SELF CARE CLUB loyalty program
1. PURPOSE
The SELF CARE CLUB loyalty program (hereinafter referred to as the "Loyalty Program") is a loyalty program created by MP CLINIC official s.r.o. IČO 27918092, file number C 126429 kept at the Municipal Court in Prague, registered on July 25, 2007, with registered office in Bořanovice - Pakoměřice, Družstevní 110, district Prague - East, zip code 25065 and administered at the MP CLINIC branch (hereinafter referred to as "MP CLINIC").
These terms and conditions (hereinafter referred to as "OP") are concluded between the members of the Loyalty Program (hereinafter referred to as "Members") and the entity MP CLNIC (hereinafter referred to as the "Participating Entity"), and define the conditions under which Members use the Loyalty Program.
OPs are accessible at any time in electronic form via a link on the website of the Participating Entity.
2. PROGRAM MEMBERSHIP
The loyalty program is reserved for persons over the age of 18 who have confirmed their consent to the OP and provided the personal data required in the application form.
Membership in the program requires no purchases and is completely free. It is limited to one membership per individual and per email address (same last name, first name, email address).
If MP CLINIC discovers that the same person owns multiple loyalty accounts, it reserves the right to merge all data, points and other benefits of the Member into one loyalty account and deactivate the other accounts.
A person can sign up for the Loyalty Program on the MP CLINIC website and at the branch. When joining the Loyalty Program, the Member is asked to fill in his personal data on the website or to provide this data at the reception desk. By registering, the Member confirms the processing of this personal data.
3. BENEFITS OF THE LOYALTY PROGRAM
The Loyalty Program allows Members to enjoy certain benefits that entitle them to various rewards that they can receive from the Participating Entity based on:
payment of the service at the MP CLINIC branch
other actions (as specified in the Loyalty Program) carried out at the Participating Entity.
Benefits and rewards under the Loyalty Program depend on the Member's status, which is determined by the number of earned loyalty points (hereinafter referred to as "Loyalty Points").
The member receives Loyalty points for each transaction, in the ratio of 1 POINT = 500 CZK
